What is Coronary Artery Bypass Grafting (CABG)?
Coronary artery bypass grafting (CABG) or heart bypass surgery, is a surgical intervention done for restoring blood flow to certain portions of your heart that are devoid of blood supply due to artery blockages or narrowing.
Such blockages can result in restricted or no blood flow to the heart or certain heart portions, ultimately causing heart attacks or heart attack-like symptoms.
In CABG surgery, blood vessels from other parts of the body are used to create a detour around blockages for restoring the blood flow to the heart.
Why Do You Need Coronary Artery Bypass Grafting (CABG)?

How to Prepare for CABG Procedure?
Prior to coronary artery bypass surgery, you might need to adjust your medication schedule, activities, and diet. You will undergo certain heart tests and blood tests the days and hours before surgery. These heart tests are:

Step 1: Electrocardiogram (ECG):
It is done to measure the heart's electrical activity.

Step 2: Surface Echocardiogram (Echo)
This is a non-invasive test that examines heart valves, heart chambers, and overall heart function.

Step 3: Heart Catheterization (Cath)
Angiography is done to examine blockages in the coronary blood vessels. Before this procedure, you should not eat or drink anything. After the procedure, you will need someone to drive you home. It takes around 4-6 hours to recover from the use of an IV contrast dye.

Step 4: Imaging
Computed Tomography Scan is done to assess the aorta, lungs, and other organs. Cardiac Magnetic Resonance Imaging (MRI) is a detailed test that examines heart structure and function. For MRI, you must remove all metal jewelry before the test. Cardiac PET scan uses radioactive tracer to identify diseased or blockage in blood flow to the heart.

Step 5: Stress Thallium Test
It is an imaging test that shows how well blood is flowing in your heart while exercising or at rest using a radioactive tracer. It’s also known as the nuclear stress test.
How is Coronary Artery Bypass Grafting (CABG) Done?
Following steps are involved in the procedure:

Your doctor will determine whether to perform off-pump surgery (where the heart continues to function during the procedure) or on-pump surgery (where the heart is temporarily stopped) based on your specific condition.

After being deeply sedated, a breathing tube will be inserted into your throat and you will be connected to a ventilator to facilitate breathing throughout the procedure.

Your doctor will make incisions in one or both of your legs or one of your wrists after setting all the tubes and monitors. This is to get to the blood vessel grafts. These grafts can also be taken from the chest area.

An incision will be made in the middle of your chest, extending from slightly above the breastbone to slightly below the end of your chest. The doctor will then cut the sternum (breastbone) in half lengthwise. To expose your heart, they will split the breastbone and spread them apart.

During the procedure, the doctor will attach one end of a graft segment over a small aortic opening and the other end over a small coronary artery opening directly below the blockage. Depending on the number and location of your blockages, you might require multiple bypass grafts.

As blood flows through each graft after the procedure has been completed, the doctor will closely inspect them to make sure that grafts are working properly. After the procedure completion, your sternum and incisions will be closed.

A: Before the operation, stress and exertion should be kept to a minimum. Family/caregivers can help the patient in this regard. At the end of the surgery, family members are counseled regarding the operative procedures. Post-operatively, patients are kept in the hospital for 5-7 days.

A: During the hospital stay, light chest exercises and some movement should be done under medical supervision. After discharge, patients need to follow a strict diet regimen, a regular exercise routine, and follow medication as prescribed. This will help in faster recovery and prevention of heart disease.
